Filled up I765, 2 ADIT style photos with A# written at the back, Copy of I485 receipt, check for 120 in favor of INS.
Walk into your local INS (at your convenience) and pick up an application if you want to send the blue signature card (optional) or download from the site above. Write A# and receipt numbers on all documents(especially in check) except the application form.
