I think that is THE latest version.
When I had filed, 9/26/00 was the latest. But if you check that form, then you will find that in Part IV you have to either check for CP or AOS.
Since this is the only form available, I would use this form. But check with the lawyer. My suggestion is, check which version of form your attorney is using? If he is using the old form (which does not have such an un-ambiguous question), then tell him there is a newer version. He should then check out about the expiry of form.
