Motor Vehicle department often register people automatically. I personally believe many of us were registered without knowing it
so we can simply answer No on N-400 In your case, you already know so you may have to explain

How "completely wrong" is the last name? Is the last name connected to you in any way ... is it actually your middle name or a name you previously used before changing to your current name?

If it's not connected to you in anyway, and you know you didn't sign the voter registration papers, don't say YES to the question about registering to vote. If the interviewer somehow digs up that card they'll see it's not your name.

it looks like my last name misspelled... the address and first name are correct...so should i go ahead and cancel the registration or just consider that it's not me and not mention at all..?
Thanks

Don't mention it, since you know you didn't register yourself. If the interviewer knows about it, point out that you didn't register yourself and you wouldn't have misspelled your own last name.
