visit visa for USA

i asked in my other post for how to apply for visitor visa for usa from pakistan..........i have download the non immigrant visa form DS 156 for it.....what else do i need? is it possible if i prepare all the documents and mail them to islamabad US embassy?
how much all this visitor visa process cost?

You can fill them in and your mother can hand carry them to the consulate. I believe it is $100 for Pakistani passport holders.
