For visa revalidation I need to fill-up form DS-156 and there is a question

35. Has Your U.S. Visa Ever Been Cancelled or Revoked?

Actually when I got my visa stamped through Company-B for the first time then there was already one H1B stamped through Company-A on my passport. Though I never used the H1B stamped through Company-A to come to U.S.

I observed recently that the visa stamped through Company-A has been marked in RED as "REVOKED UNDER SECTION 221(i) CANCELLED WITHOUT PREJUDICE" . I am not sure when was it marked , either at the port of entry or at the Consulate when I got my visa stamped through Company-B.

Please suggest if marking yes to the above mentioned question make some negative difference to the stamping or will I need to put some clarification for this on the covering letter.

---- On Sheet A explain that your earlier Visa stamp was "REVOKED UNDER SECTION 221(i) CANCELLED WITHOUT PREJUDICE when you got the new Visa stamp from XYZ consulate on date ABCD
it is not going to be problem . DOS also cancel the previous visa stamp when you get new visa stamp revalidated in US and they stamp” CANCELLED WITHOUT
