VISA Revalidation- Employement Letter

Hi,

Can someone pls tell me in what exact format the employement letter should be for H1B1 VISA revalidation?
I am applying for H1B and H4 revalidation. Should this letter contain any information regarding H4 candidate.

We need only one photograph for DS-156 right. I am asking this just as on murthy.com it says you need to send 2 photographs.

Also should we need to put any covering letter or something like that.

Pls.. i am sending this first time and doing by myself.

The letter must be quite detailed. It must describe the employer, and the employee's duties in detail. It should also request a visa stamp since Mr. x would need to travel out of the US on business matters, and also equest an H4 for the spouse. Letter must be on official letterhead.

Most people only send one photograph.

Covering letter is a good idea with a boldface fonted subject line.
