Visa for Infant

I hold a valid B1 visa for which I have traveled to the US on business and company related matters. My wife holds a B1/B2 visa, which was issued as she would have accompanied me and seen the US. Our child was born in October 2008. According to business needs, I may be required to travel to the US sometime soon. Consequently, my wife plans to accompany me with our child to see the US as well as visit distant relatives.

I had a few questions regarding this matter:

1. What is the minimum age that is required for a temporary non-immigrant visa?
2. Can we apply for our infant child?
3. We are in the process of applying for our child's passport. Indian authorities allow including the child's name to either Parent's passport OR issue a separate passport for the child. What are the bearings of either case on getting a temporary non-immigrant visa for our child. What category should we apply in?
4. I am assuming that the Parent would need to accompany the child if an interview is required. Please confirm.
5. What are the advisable and additional documents required apart from D-156 and D-157?
 
A1: If the child has his/her own passport, a visa is needed
A2: Yes
A3: Get a separate passport and apply for the child's visa
A4: The child is two months old, you expect an interview?
A5: Those two forms should suffice
