Visa for a 4 year old - apply for visa in the US

This is the issue:

My aunt is an US citizen, her daughter is an US resident. Her daughter had a child in Mexico, the child is 4 years old now. My aunt wants to get a visa for the child to visit her in the US or to bring the child when mom comes to visit my aunt. The consulate denied my aunt's daughter the visa for her child and the consulate in Mexico told my aunt that she needed to file a form in the US at the immigration office. We went to the immigration office in the US and the clerk told us that the form filled out is not the correct form, then she sent us to the US Department of State web site for more information on how to obtain a B2 visa.

Here are my questions:
1. Where, here in the US, can my aunt go and apply for a B2 visa for her grandchild?
2. Can my aunt apply for a visa for her grandchild even if her mom is resident?

A1: B-2 visas are applied for at US consulates and Embassies. No such process inside the US.

Is child with its mother? If the daughter has been living in Mexico for the last 4 years, she has most likely lost her LPR status. If she has not, the child must be filed for as an immigrant since it is unlikely she would return to Mexico.

This family needs a consultation with an immigration attorney - or at least Catholic Charities social services - to sort out their actual situation.
