I have confusion in filling up the PR card form please help me; I am working in US on H1B and reaching Toronto (Mississauga) on Feb16th Evening by Air to apply for Canada PR card, I landed as Canada PR in April 2002 then stayed for 5 days in Canada and came back to US.

I have confusion about:

Part B
13
As I became Permanent resident in April 2002
I will fill up address history after I became PR
(I will write my US address, I visited India for 2 months should I fill up that also?)

14 (I will write the Employer name and address but my Employer who is holding my H1B his head office is in Michigan but I work at client site in MD for more than a year, and I stay in MD what I should fill up here)

15 (I will count the days from my landing date till the date I notarize my papers IN Canada and I am IN Canada at that time or I should count the days from My landing date and when now I came to Canada to apply for PR card IN Canada)
Option 1 and option2 and 3 I am writing N/A
Option 4 N/A as I did not had any returning resident permit

16, 17,18 (I don’t have any problem filling up these)

G ( I will get it notarized and the reason I am going to write in blank space is that” it is less than 2 years since I became a landed immigrant, hence unable to obtain a "Guarantor” )

13. Fill up every Month since Apr 2002.

14. I dont think its a big issue. You can mention Your company name and the location can be where you are currently working. IF at all they ask for proof (which I have not heard till now) you can provide the details.

15. So you have stayed only 5 days prior to this trip. So fill that up. You should count from the landing date to the date you notarize...

Since as per new lawas you have still maintained your Residency, you dont have to fill up Option 1,option2 , option 3, Option 4.

Section G - Reasons seems OK.

Remember the dates in Section G should be same as date in the next section "Declaration of Official" and that you should sign in front of him/her...

Sign the IMM 5455 (supplementary form) in Black Ink (I almost made the mistake of signing in blue).

13. you have to fill up only the dates after you landed in Canada after April 2002 5 year history not needed, if you stayed at one address only then fill up the date XXX to date now you reach Canada

14 "mercury6" is correct, you can write Employer name and city in MD, but as your will write your MD address in 13 and Employer address in 14 it will confuse them better is to write the same address in 14 or write in seprate page and clear that your Employer is in MI but you worked in MD because od job contract

15, "mercury6" is correct but you can count the days from April 2002 landing date to the date when you reach Canada that will be 16 feb as per your info and count the days because you will be in Canada on 16 Feb, Landing date to Feb 16 count the days

9 and 10 I dont know if you should fill up " Your mailing address in Canada column or your residential address in Canada' if you have friend or relative you should fill up " Your Residential address in Canada column" becase you will be resing there.

I hope other friends will guide you more about this coulumn becase you seem to be going back to US after applying PR card
