Using refugee travel document caused problems preparing I-485.Please HELP!

Hello,
First I'd like to say sorry for posting this here but the forum about form I-485 doesn't accept new posts right now.

So I've got the following situation here:
I was granted asylum about an year ago and now it's the time to submit form I-485 to register permanent residence.
The problem is that I already traveled out of the country using my travel document in january 2010 and now I am not sure how to complete some parts of I-485.


The real problem is part 3:

Place of last entry into the USA- that's-OK!
In what status did you enter-asylee-OK!
Where you inspected by a US Immigration officer-yes-OK!

NON IMMIGRANT VISA NUMBER-??????????
DATE VISA ISSUED-?????????????
CONSULATE WHERE VISA WAS ISSUED-??????

I mean, I've got 2 nonimmigrant visas issued years ago, but the last time I entered the USA (january 2010) I used only my passport and my travel document...I suppose all these questions concern my last arrival, so should I leave these sections blank?
The other question that bothers me is:
Have you ever applied for permanent residence in th US?
The answer is "yes".
So, last year when I was approved for status I intended to apply for employment authorization card and travel document immediately, so I made an appointment with an officer at the local Immigration office just to make sure that was the right time,the right forms,etc. I was told to apply I-485 instead because the price included both other documents and I was eligible to do so at this time (?!)...So I applied and my papers were returned, because the 1 year anniversary of my approval, hadn't past.The letter said I needed to wait one year until I could apply again, so what should I put as "final disposition" in this section?I was thinking of applying a copy of the letter they sent me when they returned the papers...

Just put N/A for those questions. Make sure you are taking into account the days you were absent from US. The 1 year residency requirement is referring to physical residency, so if you were away for 21 days, you need to apply 21 days later than the original 365 days residency requirement.

just say yes you have applied and the application was returned and explain why.
