Using AP to leave US and coming back

My wife is about to use AP to go home for a short visit. Will the current white I-94 card be collected at the airport (it was H-4 when she first came to US)? When she comes back to US with AP, will the boarder officer issue a new I-94 card? What other documents do you suggest her to take with her?

Correct I-94 to be surrendered, fill out new one on re-entry. Suggest carry all documents (copies) regarding immigration , EAD card etc,
