USCIS error in printing name : please advise

I received my card few weeks ago. In my last name USCIS has put both maiden name along with my current last name.
It's as below
"Current_last_name (space) maiden_name (space) First_Name (space) Middle_Initial"

I called them and they asked me to fill up I-90 form and send it to the center, which processed my application. I have a question that do I need to send ORIGINAL green card along with I-90 form or photo copy should be enough? I feel hesitant to send original card. Please advise.

I am going to send following other docs
1)Copy of Passport (which has correct name)
2)Copy of Driving license (which has correct name)
3)Copy of credit card (which has correct name)
4)Copy of marriage certificate.

Do I need to send any other docs besides above? Please advise.

Hi, you posted this on the wrong thread - the right thread where lots of information on situations like yours is available is the "Physical green card" thread.

Is this a clerical error and do any of your other documents (birth certifcate, marriage certifcate, change of name affidavit after marriage, passport) show your correct last name?
