USCIS error in name printing: please advise

I received my card few weeks ago. In my last name USCIS has put both maiden name along with my current last name.
It's as below
"Current_last_name (space) maiden_name (space) First_Name (space) Middle_Initial"

I called them and they asked me to fill up I-90 form and send it to the center, which processed my application. I have a question that do I need to send ORIGINAL green card along with I-90 form or photo copy should be enough? I feel hesitant to send original card. Please advise.

I am going to send following other docs
1)Copy of Passport (which has correct name)
2)Copy of Driving license (which has correct name)
3)Copy of credit card (which has correct name)
4)Copy of marriage certificate.

Do I need to send any other docs besides above? Please advise.

get the copy of I-485 form that was submited by lawyer (ask the lawyer for the copy of I-485 form that was filed) send the copy and you will also know if he wrote correct name on Form I-485
if you had EAD or AP in past send the copy
send the copy of I-485 filing reciept
send the copy of any reciepts or documents that have correct name on it
**** make a cover letter and explain the mistake on the Card and also write what should be on the card so that it is correct and match with your all other documents. on top of cover letter write your A#, your correct name, address and sign the cover letter with date
****** keep the copy of each document including I-90 form that you are sending.
