Where could i get the I140 premium processing form

I checked the uscis website and i could not find the form, please help me. My I140 has been delayed.

It seems slightly biased though.
Does anyone knows if EB-2 Skilled Worker is included or excluded from the premium processing.

The only exclusion seems to be EB-2 NIW, but there is no mention of EB2 Skilled Worker.
