Hello, I have a few questions about my situation and im hoping that i will find some answers.
I'm 28 years USC who's dating a 44 years old woman who was on F1 Visa and she's been out of status for 12 years. we've been dating for 18 months and we're planning to get married next month and then apply to the USCIS for a green card application, I have some questions regarding the process. Im mainly worried if the age difference is considered a red flag? Also we're not planning to have a wedding since our families are overseas so im not sure if not having any wedding pics will be another red flag? we do have lots of pics though. Im planning to add her to my medical insurance, life insurance, lease, bank join account as well as a beneficiary for my pension plan. the last question is about her visa status, is that considered a red flag since she overstayed since 2001? All answers are appreciated.

I can try to answer a few questions

It may be an issue. It's somewhat unusual for a 28 year old guy to marry a 44 year old woman. Make sure to have lots of evidence of bona fide marriage.

I'm not sure. But to get married you are going to have to have some kind of ceremony, even if none of your family will be there. So at least there will be pictures from that.

the last question is about her visa status, is that considered a red flag since she overstayed since 2001?
No. It's not an issue.

How long did she attend school before losing her F1 status? Did she graduate? For the interview she'll need to bring proof that she actually attended school for at least a semester (preferably a year). If she graduated, she should bring the diploma, otherwise bring the transcript.

she attended school for one semester. and she didn't graduate.

I hope she did enough of attending school and studying in that one semester to actually pass at least one or two classes. Otherwise she's at serious risk of having her green card rejected and getting deported for student visa fraud.
