I-94 attached: it has two parts - left one is for personal record and says valid from 04/21/2006 to 03/06/2009. The right side part is just like I-94 we get in the port of entry without any date marked on it except my birthdate.
----------------keep the copy of approval notice(front and back) in your immigration file at home and then detach the right side I-94 and attach in your passport
Can you tell me what really this new I-94 mean to me that says valid from 04/21/2006 to 03/06/2009 ?

*** when you enter US you can show Valid visa through A AND new H1 employer approval notice at POE and you will get new WHITE I-94 till 03/06/2009

I have further question on the same topic: do you think VISA stamping with new employer B is still not required (having a valid VISA based on previous employer's H1B) even though the previous employer revoked the H1B petition after I left the company (which I don't know whether they have done it or not).
---Employer has ONLY revoked your H1 approval by writing to USCIS. Visa stamping is not under USCIS it is under DOS. you can enter with valid Visa stamp of prevoius employer and show valid H1 of B at POE. you can also cantact your lawyer for more info.
