I am going to file in June for my wife, she is currently on H-1. I understand that I have to send the I-130, I-131, I-485, and I-765 to a Chicago Lock-Box. I understand that I have to send our photographs, our G-325A, and application fees with these forms. I have the following questions:
(i) Can I just send one money-order or do I send four different money-orders for each of the forms? Will a personal check work, or I need to send money-order?
(ii) What is the address of the Chicago Lock-Box?
(iii) I understand that I have to send a medical results envelope with the I-485. What other documents do I send with this package?

I understand that the I-485 will be transferred to the local INS office for an interview, I-131 and I-765 will be processed by the National Benefits Center, who will process the I-130?

About first point: the disadvantage of sending one check/money order is that if you make an error in one of the forms and they have to send it back, they send all four forms. If you write 4 separate checks, only that form will be sent back to you. Also, personal checks have the advantage that you can track when they are cashed.

Ans: You can send one money-order but its worth sending seperate personal check/money-order for each form. That way if any of the form is rejected only one of the form would be sent back but whole package would not be rejected.
Yes you can send personal check but better write it from your account showing your name & current address.

(ii) What is the address of the Chicago Lock-Box?

Ans: Dude, its there on the front page of I-485 !!!!!!!!

(iii) I understand that I have to send a medical results envelope with the I-485. What other documents do I send with this package?

Ans: Dude, how can you forget the most important sponsorship form I-864. Also send your wife's birth certificates, I-94. Again follow instructions on I-485

Ans: I think your local DO office will process I-130. But approval of I-130 is not important, getting your wife's interview on I-485 is important. And that is sure gonna be processed by your local DO.

I found all instructions in the form I-485. One more question on the form I-485. I am going to be filing the I-130, I-485, I-765, and I-131 concurrently, so do I check box a in Part 2 of the I-485 (indicating a relative visa petition is filed with the I-485). I will not have the approval of the I-130 when I file the I-485, hence I ask this question.

Yes, check box a.
By the way, the service center responsible for your area will process your I-130, not your DO. Check out the official website of USCIS: www.uscis.gov. There you can see which service center processes your petition.
I am pretty sure that you will have to send 3 separate checks or money orders for I-130, I-1485 and I-765. You can include the $70 biometrics fee in your I-485 check.
