In Form I-131 under NIW, which one should i choose between one trip and more than one trip? And how to explain my qualification for AP? what is the main point?

1 trip only give you 1 entry AP and other give you multiple entry AP

Thats correct ask for multiple entry.

And Reasons can be explained :
issues
1. pending I-485
2. Reason to visit : mainly visiting parents back home
3. Medical reasons.
