URGENT - Please Advice - Regarding Missing Other Names

Here is my wife's situation, My wife's middle name has 4 names/parts to it, her full name looks like (Fname, Midp1 Midp2 Midp3 Midp4, Lname) , when we applied for her H-4 visa to come to the US and join me (I had H1-B at this time), the visa application had her full name (Her passport also has this full long name).

During the process of my GC (I-485), My company's attorney who was handling our case, let her sign some document which says that from now on (Fname, Midp1 Midp2 Midp3 Midp4, Lname) will be represented as this short name (Fname, Midp1, Lname) and from then and all of her forms filed with USCIS AP, EAD, and GC, SSN, Tax forms all have this short name (Fname, Midp1, Lname).

Now we sent her N-400 form and we put her name as (Fname, Midp1, Lname) in Part A & B and we did not put the full long name (Fname, Midp1 Midp2 Midp3 Midp4, Lname) in the Part C OTHER NAMES section of the N-400 thinking that as she already signed that doc during GC process, we do not need to mention it, but we just get to know that we should of added this full long name for FBI name check , as her H-4 Visa & Her home Country Passport has this full long name .

Her N-400 Interview will be on 11/10/09 and we do not know how to tell the IO about this mistake and that we did it not intentionally, and what might be the complication of not having this full long name in the N-400 application when we send it

Just mention to IO that she had previously used an alternate name. The interview gives her a chance to update things on your application. Expect her case to be delayed since it will have to be sent back for an additional name check.

I have the same issue. I omitted my long middle name when I applied for my GC. Just had first and last name on GC application and got approved that way. When I applied for N400, I listed my full name as it appears on passport as legal name and then listed name that is on GC. When I went for finger printing, the people there saw the difference and then added my full legal name to alternate name when they took the finger printing. I still think that FBI will do all kind of matches when they do the name check. Middle name or Initial was optional for GC few years back and from what i hear it is a must now. I will update you on how this went with my interview which is on 10/27/2009.

Was the "some document" an official name change document? USCIS will treat her original extra-long name as her current legal name, and put the extra-long name on the naturalization certificate, unless there is an official document proving that she changed to the shorter name.

However, if she does not have an official name change document reflecting the new shortened name (Fname, Midp1, Lname), she can request to have her name officially changed to that shortened name as part of the naturalization process. Then the naturalization certificate will have the shortened name.
