URGENT: I-134 Affidavit of Support - Send to where?

Hi, I have a relative in the States that is doing the I-134 Affidavit of Support for me. He had gotten it notarized, but do not know where to mail the Affidavit of Support form and supporting documents to.

On the USCIS I134 Website, there is a clause below :

"Once the form is affirmed, it should be mailed to the same USCIS office where the application was filed."

May I know which USCIS office should he send it to?
He resides in California. My DV2009 case should be current in December. I am from Asia. We have never filed an I-134 before.

Please help as I need to let my relative know as soon as possible.

You need to bring it to the embassy where your interview is.

If you are doing CP, then get your relative to send the notarized I-134 form together with the supporting documents to you.Once you receive the documents , take them with you to the Embassy on the day of your interview and submit it on request.I hope this helps.

My sponsor initially thought he had to send it directly to the USCIS.
