Urgent Help on BC

Hi,

My spouse Birth Certificate has all the correct information except that the last name is not mentioned..only they have printed first name. But in the passport both first and the last name is mentioned. Is it going to be problem ?? Do we need to change the BC to reflect the last name as shown in the passport ?? or do we need to take affidavit from parent and family members mentioning first name and last name.

Pls advice.

Do i need to take the non-availability certificate from the local authority and then affidavit from parents..or just need to take the affidavit from parents, since we have already the Birth certificate..except that last name is mention.

my BC doesn't show my name. But I can not take non-availability since it is available. Other discussions strongly suggested that we take 'available incomplete BC and affidavits' . Thats what I am doing.

How do we get the 'available incomplete BC'. Is there any specific format which is need or is it a standard format available in the govt. office. Also, do u know what does the affidavit should say!! Pls let me know..so that i can work on it right away..since my interview is on 21st Nov.
