Urgent Help from Friends for filing 485 ?

Guys,
I need urgent help for followings:

On for I-485 first page:
I entered Date of Last arrival: (2006) and I-94#.
Next line asks;
Current USCIS Status : H1B
Expires on(mm/dd/yyyy) : ???(whic is 2005) This field is the date on I-94 (whic is also the date my H1's stamp on passport) or the date on my H1 Visa extension paper, which is 2008.
Note: I got extension approval last last year but don't have stamp on my passport yet.

What about "Consoludate where visa was issued"

I know one friend put Department of State.

When I check my I140 petition, which has been filed in 2006, visa expires dated as visa stamp. I confused now. Please comment!

If I enter approval date, which is 2008, should I also change the filed, "Where visa was issued" as the name of consoludate or change as Department of state ?

My H-1B situation is similar to yours (am currently on year 4 of H-1B extension but don't have visa stamp in passport).

For the I-485 application, my attorney entered my current H-1B visa expiration date but for the 'Visa #' and the 'Place of issue' he entered the details from my expired H-1B visa passport stamp.

I have certified translation of my Marriage and birth certificate.

Can I send photocopy of them or should I send originals on which you can feel the stamp ?

How about G-325 for kids under 10 ? I think it's not necessary for them

Last one is how to sign 485 forms by kids under 10 years ? We sign for them or they do (like they print their name)

My lawyer had me notarize the translations. Anything like this will need to be notarized. You can get it done in a bank. If it does take time send it in. Not worth loosing time on this.
