Urgent !!! Format of Non-Availability Certificate for Date of Birth Needed

Hi All

I got an RFE from INS to submit Employment letter, recent PayStubs and Non-availablity for Birth Certificate. I have the first two, but need the format for Non-Availability Certificate for Date Of Birth.
I need it reall urgent. Guys, you can understand the urgency. Please help me and provide any link that could be helpful in this.
FYI, My RD is 05/16/2K1 and ND is 07/19/2K1. I already got the AP, EAD. FP was done on 10/20. I am on PCee\'s list.

Thanks a lot rsgrc and kotag kris, this was really very helpful

I will try to expedite the whole process so that I get it pretty soon. And wish that the Attorney replies to the RFE soon. He got the notice today. I just pray that things are fine after the RFE is replied.

  Did u contact ur people in india abt this?if so what do we need to send from here to get that from BDO office.do we need to send our 10th class certificate or anything?
