I am not so sure what you are asking.
If your question is:
Are there areas specifically designated to be underserved for particular specialties ?
the answer is:
No. Health care personel shortage areas and medically underserved areas are designated based on the ratio of primary care physician FTE's to population.
(The only exception is psychiatry which is counted under the 'mental health personel shortage areas', not under 'primary care'.)
If your question is:
Can medical specialists obtain waivers of the foreign residency requirement based on service in an underserved area ?
the answer is:
Yes. Either under the Conrad30 program. Many states these days will sponsor specialists if a hospital or health care facility indicates that they have a need for this specialist. Also, the Veterans administration can sponsor specialists.
