Hello guys, been a while since I've been on here! Had two quick questions, I am filing a 751 waiver because my marriage did not work out and resulted in a divorce.
First question: Part 3, question 9 - Have you resided at any other address since you became a permanent resident? (If yes, attach a list of all addresses and dates.)
Shortly after I received my green card my wife and I moved into another apartment, I filed the change of address form right away. I am still currently living at the same address. Should I still include a list of my two addresses even though I already submitted a change of address in the past?
Second question: Part 4 - Information About the Spouse or Parent Through Whom You Gained Your Conditional Residence
Were it asks for my wife's "Family Name" should I put the name that she had before I married (maiden) or the name she used after we married? When we got married she added my Last name to her maiden last name, for example if her last name was Smith and mine was Jones, he added the "jones" Jane Smith-Jones. When I filed, her name was listed as Smith-Jones on the paperwork, so I'm assuming I'm using that name on this form correct?
