Tricky RFE - Experts Please advice

There are 2 parts to my RFE
Part1 Says : Re-do the I - 693 Supplemental form since the Physician used the wrong one initially. We cannot accept it

Part2 Says : Submit documentation to show that the Physician who the Physical has been recognised by the Service as a designated civil surgeon.
( The problem here is his Clinic name is on the list bu tnot his name - how to prove this point ) - If this is not the case Please submit a completed From I693 with Supplemental with a designated physician

What are the best options
1. Resubmit the new I693 supplement along the with the Old I 693

2. Is it better to just go to a new Physician and just submit all fresh

IMHO, go for the 2nd option even if it means a little bit of
delay...that way you don\'t have to \'prove\' anything, and give an opportunity to VSC to try and \'disprove\' that...I think you don\'t have to go through the process of taking injections again, since you have already completed that and the first doctor would vouch for that...
