translation of doctor's note to spanish consulate

I have a doctor's note, it needs to be translated from english to spanish. I am submitting it to the spanish consulate. I am in the USA. Does the translation have to be notarized? apostilled?
 
There are many good companies in the USA which provide good translation services. Before choosing any service for translation one should inquire whether they accept documents through mail, fax or online as an email attachment or as a scanned document. Find a company that has a service procedure convenient to you.
 
It depends. For something that's not a government official paper, like a doctor's note, I'd just translate it myself and add a snippet at the end guaranteeing the accuracy of the translation. I'd check with the embassy first though.
