translation issue !!

Hi,
My birth certificate is in Hindi. As per the application:
C. Translations. Foreign documents must be translated into English and "certified" by a competent translator (see format below)

CERTIFICATION: I, (insert name), hereby certify that I am fluent in the English and the languages and that the above is an accurate translation of the attached document issued by .

Date:
Signature, name and address of translator

I have mine translated & attested by a Notary from India. Is it mandatory to have it in the format above?
What is the quickest way to get this done? Is there any place in the US where I can get this done?
 
this is what my lawyer said:

I had a similar issue with birth certificate of my wife, which was in kannada. He said to ask any of my kannada friends to prepare a translation, write his name and address on the translation and sign it. (I prepared the translation, by making a copy of the original and writting in english, above the kannada words).

My lawyer accepted the same, and I do hope that it works with the INS

I did the same

My lawyer said it is more than enough... I also got affidavit from my parents defining my birth details attested by notary office in India on Rs.20 Stamp paper
