2nd FP 5/22/04, updated on 5/28/04
RFE issued 6/03/04, same stuff (W2, tax returns, pay stubs, EVL)
RFE received 7/01/04
AD 7/15/04
LUD : 5/7, 5/14, 5/18, 5/28, 6/03, 7/1, 7/3, and finally 7/15/04
