TO DO for EAD renewal

All gurus,

I have several questions:
1. I am working using EAD now, should I include I-94# from my H1B? ( for column 10, I saw my lawyer filled out my I-94# on my first EAD)
2. My lawyer use Alien number which is not Alien number from EAD, I think the Alien number comes from Receipt notice from I140? Is that OK?
3. My current status is Adjustee, not H1B holder, can I say in column15 on application that I am an Adjustee of Status based on employment EAD
4. What documents should I include? I saw some people includes 140 approval. Here is the documents that I think should be included. Correct me if I am wrong:
- copy of EAD. Should we send original?
- I-485 Receipt Notice
- 2 photos
- I94 document. Do I need this even I do not use H1B anymore?
- a check 120 dollar? Do I have to write to BCIS or I have to spell out the BCIS extension?

I just renewed my EAD.
I have sent these docs.
.Copy of ur 485.
Money Order for 120$.
Copy of ur current EAD..My H1 B is also epired,so i put AOS(Adjustment Of Status).
2 photos.
A small letter requesting for Renewal of EAD
That's all I did .

which is ur service cernter?.If it CSC then it does not make a difference whether you send it thro FedEx or bt mail.I have sent it thro ordinary mail.
Regarding letter,write a simple letter to the service center requesting for the renewal of EAD .
That's it.
