TN Mail-in question, Form I-539

Can anyone please clarify a question or two for me pertaining to Form I-539 in regard to TD dependants applying by mail for an extension of status.

Question 1) Is Form I-539 "soley" for the TD spouse & kids, or does the TN holder fill out the first part and add the TD spouse and children at the end of the form?? (although the actual TN is filing I-129, Form I-539 is not clear to me)..

Question 2) (Part 6. of the application, )should the employer sign this ? (technically the employer is filing this form? correct? the application is not clear.

Question 3) Do Nafta Canadian TD dependents have to pay the fingerprint fee?
