One last thing should I fill the mailing label and send it or just like that. Got this question because I am not able to type in it using Acrobet Reader like the rest of I539.

Regarding my visa I have got the visa on my passport till Dec 2003 and the I797 which I rec is till Dec2004. So in this case also do you think I have to send it 6 months before.

-----You are not clear, you have H1B approval notice valid toll December 2004 if you want H1B extension then your employer or lawyer has to get new LCA from DOL for you and can send H1B extension 6 months before December 2004 to BCIS.
Consulates or Department of State stamps visa sticker. H1 or H4 visa STATUS is extended by BCIS, don’t try to mix Mahabhart in Ramayana. Understand the difference between Visa sticker and Status extension; use the proper language so that others can understand your issues
---- If you want to get new H1B visa sticker in passport valid till 2004 then you can send your passport to DOS any time in December 2003 or after that (it should not have expired more than 1 year.
