The newest i-751 form ;August filer

Hello everyone,
I am new to the forum and I am preparing the package to submit my i-751 ROC.
I see online everywhere to see how to fill out the form but for the new version of the form, there are some stuff slightly different...
My confusion starts from... how to treat the blank boxes.
I see people are saying NOT to leave it blank but in the instruction of the new version says that :LEAVE it blank if not applicable.
Anyone who filed after June 2013, please let me know how you did it and how it went?

Also Part 4 list all the children.
I just had a baby with my husband who is US citizen. so the baby is automatically US citizen now.
I am the only one who is permanent resident.
Do I list my baby on the list?
Some says yes to list a US citizen but Others say no you only list if the child need to remove the condition as well.
I am confused because the box 12 says if the child is applyin with you or not and you can say no.

oh any one more thing,
how do you pay the fee? it says check or money order but the check means the personal check or cashiers check?
I prefer the personal to just keep it track..... but not sure if they accept?


I really do not want this to be the problems and not want to be denied!
I need to submit it like today because mine expires in September 22.

I have all the documents to be submitted with my I-751 application, but I am confused as of who petitions for the removal of the conditions on my resident status. It is my husband in my name or myself? Of course, it's is a joint application. But who addresses the USCIS officials, my husband or myself?
For instance...when you write the cover letter with all the supporting documents, at the end of the letter it's " Sincerely, my husband's name or it's my name". I hope I am not confusing you.
My conditional GC expires September 20,2013.

My cover letter was signed by both my spouse and I. So it ended, "Sincerely John Doe and Jane Doe" and we both signed. Your husband is the petitioning spouse. Hope this clarifies it.

Thank you I saw it and never had a chance to reply ! Crazy baby driving me nuts and no time to be online at all! Lol

Today I saw my check was cashed so I hope I get a letter soon !
