The check for I-131 reentry permit should payable to whom?

Hi, Guys,

I am filing I-131 for reentry permit. Below is my cover letter, am I missing something or is it good enough. Also on the personal check, whom I should write down to payable to?

Dear Sir/Madam,

I am filing I-131 to apply for reentry permit, my last entry into US is on XXX/XX/XXXX at San Francisco. My application for permanent residency is approved on XXXXXX/XX/XXXX. I have included the following documents. Thank you very much,

1. Form I-131

2. Copy of my permanent residency card

3. Copies of my US visa

4. Two recent photos

5. A check of $165
