surname and first name interchanged in Form DS-156

Hi

In form DS-156, my surname and first name is interchanged (by mistake). How can I correct it? OR shall I go with this inconsistant information to the consulate? Are there any chances of rejecting B1 VISA with this situation. Please guide me.
