Supporting Docs for e-filed AP

Hi All,

What supporting documents do I need to send for e-filed AP?

I am planning to send the following

- Copy of the I-131 Online Confirmation Receipt notice
- Copy of Current EAD (Front & Back)
- Copy of Current EAD mailer
- Copy of EAD Renewal Confirmation receipt
- Copy of I-485 receipt
- Online Status of my I485 receipt
- Copy of I-140 approval notice
- Copy of I-94
- Copy of Passport (which has Bio-graphic data)
- Copy of Drivers License
- Copy of marriage Certificate (with wife's Application)

Is this list good enough?

Is there a template for the request letter that needs to be sent with the supporting documents?

I don't think Online Status of I485 receipt or EAD related items are required.

Also, copy of previous AP (if any).
For your spouse, add copy of primary (your) I-485 receipt.
You need to send both sides of I-94 copies.

There is no standard template letter. You can simply write that as you planning to go for vacation within one year, that's why you need AP. Therfore, you are requesting USCIS to issue AP for same.
