Supplemental form to I-693

Hi all,

I received an RFE requesting the following items:
1. G325-A
2. I-693 (medical)
3. Supplemental form to I-693

I went to the doctor; he filled out the I-693 indicating that everything was OK. In the Immunization determination part, he wrote “Immunizations up to date”. However, he did not complete the supplemental form.

Questions

1. Is this statement by the doctor a clearance for any vaccination requirement or do I still need to have him complete the supplemental form?
2. If he needs to complete a supplemental form, does he charge separately for it?

How do you know? Did he give you a sealed envelope? If he didnt give you a sealed envelope with the reports, you have a MAJOR problem. Go back and ask that he put the reports in an envelope, seal it, then sign and stamp the flap.

1. Go back to him, and explain that CIS might get confused without a supplemental. So even if nothing is required, he should still fill out a supplemental with his name/address and indicate that no information is required. Tell him a number of immigration and asylum experts told you its absolutely required.
2. He shouldnt charge extra for the supplemental.

If he doesnt give you the supplemental, run to another authorized doc. You may get a discount by showing the current reports. And then complain to the state board of medical registration and the CIS about this DOC. Also post his name to this forum so others may avoid him.

Thanks floyd. I've actually called the doctor's office. The nurse didn't give me a copy of the supplemental form, only the I-693. But she told me the supplemental form was inside the sealed envelope. I guess I'll have to trust her.
