Hi folks, my 140 just got approved yesterday so I've moved from the 140 threads to the 485 section.

Does anyone know the list of docs needed for the 485. I have a lawyer but was hoping to be a little proactive in getting the stuff together (ex. birth certificate for me and my spouse?, driver license here?, doc cert?)

Congrats Mr. Bhalla,
please let us know if your I-140 receipt has a LIN number or SRC. LIN is Nebraska and SRC is Texas. Thanks!
 
Here's all the List Of Documents when I filed for I-485:

A) Income Tax Returns for the most recent 3 Tax Years
B) W-2s
C) Proof of their current employment of self-employment

For Us Citizens or nationals( this is for USC- my husband)

A) a copy of your bith certificate
B) passport
C) certificate of naturalization or citizenship
D) marriage certificate (ur marriage ceritficate w/ ur USC husband)
E) divorce
F) bank statements covering the last 12 months or a statement from an officer of the bank

We didn't hired any lawyer to filed our papers. We did it all by ourselves. It should have some papers or check list included in your Form I-485. Hope it helps.

Documents Needed for Filing I-485

Here is the list of documents which my Lawyer asked me to send to him.


CHECKLIST FOR I-485
(The Final Step of Your Permanent Residency Application)


Please provide the following documents to our office. Also, please arrange everything in the following order. Please note that your home address which you provide at this point should be valid for two years.

___ 1. Audit Worksheet. Do not worry about Alien Registration Number because you do not have it. #3 to #6 should be your address that you will live there for at least 2 year. Changing address after filing I-485 is possible but notices from USCIS may get lost in the mail;
___ 2. Two passport photos for I-485; another two for I-765 (EAD card) and another two for I-131 (Advance Parole). Write name with pencil on the backside of each photo; (All photos have to be passport photos).
___ 3. Form I-485 (sign and date on page 3 & 4; we will prepare page 1 and page 2 for you).
___ 4. Form I-765 please fill as much as you can, Form I-131 please fill part 1 only and sign on part 8.
___ 5. Form G-325A (not required for children under 14). Please draft on a copy before you fill out this form. Leave the Items of “FILE NUMBER” and “Alien Registration Number” blank.
Note: when filling your employment history in US, please look at your employment authorization such as EAD, OPT and H-1. Make sure your statement is consistent with your documents.

___ 6. Medical Examination should be filled out by USCIS authorized doctors. Please visit http://uscis.gov/graphics/exec/cs/Index.asp?EmptyRecordset select the state, it will provide you the Dr. List. Or may call 800-375-5283, and speak with the representative, then provide your zip code for doctor’s list; or in Metro Detroit, please call Dr. Dass at 248-546-9100 for appointment.
___ 7. Form I-134 for each of your dependents (not required for your spouse if he/she is working with an employment verification letter, but we need a employment letter).
___ 8. Statement of visa status including date of entry and date of change of status, Please make a copy and attach all documents which issued by the USCIS to prove your statement, including passport (all pages) visa, I-94, I-20, IAP-66, Optional Practical Training Card, Employment Authority Card, two color copies of latest visa page or Driver License, H-1B and H-4 approval notices, F-1, F-2, J-1 waiver, etc., a entire copy of your passport. if applicable (please arrange these supporting documents in chronological order), Please note that we need 3 sets color copy of passport page (the page that has your picture on it); Also make sure you provide the copy of you I-94 for the last time you enter the U.S.
___ 9. Copy of Birth Certificate (must have your name, date of birth and parent name) if not, 2 affidavits from older relatives (parents preferred) documenting you date of birth and a non-availability certificate from the government where you were born.
___ 10. Evidence of marital status (Marriage License, Divorce Certificate, if applicable), please submit one copy for each of your family members (spouse and children).
___ 11. Employment letter from your employer to verify your employment should indicate your title and current salary (not required if concurrent filing I-140 and I-485);
___ 12. Checks payable to USCIS: For filing I-485, $395.00 ($325.00 + $70.00 Fingerprint fee) for each adult, $225.00 for each child under 14; $180.00 for each EAD card; $170.00 for each Advanced Parole; Note: We do not charge attorney fee for EAD card and Advance Parole when filing together with I-485 for the first time. We do charge $100.00 for each EAD card or Advance Parole application after I-485 has been filed. (The USCIS prefer you write separate checks for each application.).
