Stamping visa

I have visa stamp from A company on my passport which expires in Oct,2003. Currenttly I am with B company and got my new H1 till 2004, visa of the B company is not stamped on my passport.
*What is the procedure?
*If I want to stamp the Visa from B compnay on my passport how many day's priors should the necessary documents be sent to INS?
 
H1B revalidation within 365 days

I applied for a H1B revalidation at St.Louis within 365 days of my 1st one year expiration and got a responses in January 2003 that under 221(g), I've to apply from outside US. My original stamping was from Toronto - I also wrote to them and they replied by writing that it is advisable to apply from home country as consular officers at 3rd country has too less info. to judge applicants.

I've till Dec 19th 2003 for the 365 days window. As moderator mentioned, can I reapply for a revalidation of my H1B after August 2003 or visit home country within the 365 days with the notice of 221(g). Any suggestions on the implication of section 221(g) will be extremely helpful as well.
