Received the 485 approval notices for me and my spouse yesterday. The notice says to take Arrival/Departure Record, Form-I-94, passport and two ADIT style photos to San Francisco INS office. What does Arrival/Departure record mean? All my arrival/depatures were stamped in passport so what else do I need to take?

Besides the above documents, is there anything else that I should take with me.
 
Arrival/Departure record is nothing but your I-94 and in case if you have AP take that too. EAD card and approval notices. INS will take your EAD/AP/I-94 and will stamp passport.
Keep your index finger clean, I have not visited SFO lately so I can not comment on this but you may want to stand in appointment line. And since you don't have any fix time appointment then I would say go somewhere about 10AM so by that time line will be cut to short.
