Spelling mistake in Birth Certificate..pls help

Hi there,

I am all set to apply for my I485 but realized that my birth certifcate has a spelling mistake. The place i was born is slightly far and hence i want to avoid the trip if possible. I think an affidavit is acceptable but am not sure of the format and also who should sign the affadivit. Attached is the link in the consulate which talks about the issue but i cannot open the link for some reason.

it should be enough to get an one notarized affidavit that includes both parents but you could also include another one from a relative. your parents will sign the first one and relative the other one and a notary seal on both.

attached is a sample affidavit of birth for parents and a sample for relative... but make sure your attorney approves of these ssamples before using them
