On form DS-230, did you ask for the Social Security card to be automatically issued? If not, you will have to visit a Social Security office to apply for it.
Don't contact Homeland Security, you need to visit a Social Security office.
Go to a Social Security office with your green card and find out if a Social Security number was requested for you by Homeland Security. If they say yes, they should be able to tell you the number, and you can ask them if the SS card was mailed to you and when it was mailed. If it was mailed long ago, it's probably lost in the mail so ask them to issue a replacement which should arrive in 2-3 weeks.
If there was no SSN requested for you, apply for it (bring a completed SS-5 form so you can hand it in if you need to apply for the SSN).

If the social security was sent and lost in the mail, ask Social Security to issue you a new one and flag the old one. You don't want Social Security to reissue a replacement card, whereas some criminal is using your social security to rake up debt and other unmentionables. If no social was issued, then request a new one to via an application form.

They have done it, but only in rare circumstances like Witness Protection Program or extreme cases of identity theft.

Who handled witness protection program? FBI? Does FBI trust other government agencies on this
matter? A FBI agent call SSA office saying "We have a witness under protection adn we need you
to give this person a SSN and please make up his record according to data we supplied"?
When FBI gives a person a new Identify, all data about this person are faked, do other branches of teh fovernemnt want to cooperate?
if a PR is under such a program, can he get US citizenship without USCIS. FBI simply make up a new identify gor him saying he
is born in a US state
