Hello

I would appreciate a response from anyone who has applied for the US passport.


1. Money Order
If I make a money order should it be for a single amount of 237.72 (140+25+60+12.72)


2. Middle Name for Parents on DS-11
Do I have to include ALL middle names of parents when filling the form?


3. Signature (N400<-> CoN<-> Passport App)
I have usually signed as fname&lastname (no middle name in signature even current DL is like this). However
on the N400 app the requirement is Complete Signature. Also the CoN has it as Compelete and true signature
(so I had to include my Middle Name when signing both N400 & CoN).
So now when signing the passport application Is it a MUST to sign the same way i.e include middle
name?

Any input is welcome.

No. $25 is the processing fee that you pay to the acceptance facility (e.g., post office). You should write two cheques:

  • 140+60+12.72 to the Dept of State
  • 25 to the acceptance facility

Write as much info as you can. If you run out of space, then it is to your discretion. This section is not very crucial.

Put your regular signature on passport - the signature that you put on cheques, credit cards, DMV documents, etc. I have an illegible scribble as my regular signature, so I signed all my documents with it. The only exception was my Certificate of Naturalisation and the photo that was attached to it just because the IO directed me to write my full name.
