Should I count L1 and H1 years for 6 years

I worked on from Aug2001 to April2004 on L1. In April 2004 I went back to India. In Dec2004 I came back with the same company on H1B for 2 weeks only. Again with the same company I came back in July2005 On B1 for same company.
For my 6 years should I count from Aug2001 till July 2005?
Please provie me the informaiton.

When my the 6 year starts in case of extension?

I have wokerd on L1 for almost three years, I went back to India and get H1B approved and stamped from same company. If I stay out of USA for one year then I wll get another 6 years. Am I right?

Another question is when my new 6 years will start from the date I got H1B petition approved, OR From the date when I got H1B stamped on my passport? OR When I enter first day on new VISA to USA?
