Self filling AP and EAD- Need help

Hello Friends:
I have a I-140 pending and my lawyer submitted the I-485 last month. I have the LIN number for the I-485 and decided that I will file the Advanced Parole and EAD by my self and for my wife.
(1) When I file AP, what other documents are needed to file with the application other than the photographs? (eg: passport pages?). There is no clear information on the instructions for application
(2) For EAD filling what other documents are needed other than photographs and copies of previous EAD copies?
(3) My local BCIS office gave me a list of P.O. Box numbers for Nebraska. There are seperate P.O. Box numbers for AP and EAD. Should I send them to the seperate boxes or can I send both to either EAD Box or AP box?

------ FOR AP:
BCIS Fees check $110, Form I-131, Copy of I-485 receipt, 2 photographs, (Copy of marriage certificate if filing for spouse) current I-94 copy (front and back) write every document attached in cover letter with EAD or AP application in order



(2) For EAD filling what other documents are needed other than photographs and copies of previous EAD copies?
------ FOR EAD: BCIS Fees check $120, Form I-765, Copy of I-485 receipt, 2 photographs, (Copy of marriage certificate if filing for spouse) current I-94 copy (front and back), top portion that you got with first EAD and copy of past EAD (front and back)
****If service center is NSC send color copy of first page of Passport page that has photograph and date of PP issue and PP # and also color copy of driving license. Attach for AP and for EAD
** write Name, DOB,your A# and I-485 pending# at top of your cover letter and your address
