Self employed multiple addresses

Hi,
I am self employed from past 3 years and my Home is my 'employer address'.

But, my address changed 3 times during that time.

a) Should I make 3 entries in Part 6 Section B of N400 where I need to provide employer address from past 5 years

or

b) One entry with latest address suffice ?

Any time you officially changed your address - as in your tax forms or DL or state issued ID etc - the address needs to go into N400 application. Of course, temporary addresses need not be mentioned there. I was out of town for about two months back in 2009 and I didn't mention that in my app. I never got any question on that and got my Natz done two months ago.
