One of my freind is stuck in Ottawa for back ground check. He applied on 31st Jan 2005 and was asked by counsellor that his name match in the list but DOB doesn't. So they have to run the name in main data base and after approval/clearance from Washington a H1B visa can be issued.
h4 visa for his family is also denied till he get the visa after clearance. He has two US born kids with US passports.
