Second FP Documents

Guys,

I am planning to take the following documents for my second FP:

1. Passport & I-94
2. Driver's license
3. FP notice
4. I-485 receipt notice and I-140 approval notice

Am I missing anything?

Also, do we have to fill up any forms there? If so, what kind?

no, that's enough, you are all set.

even if you take only passport, drivers license, FP Notice
no problem. because all they need is identification and FP Notice.
 
Sorry, I didn't read your second question. Yes, you need to fill out a form there that asks for personal information like name, date of brith etc... You need to know your SSN (my wife didn't remember her SSN, so we had a bit of struggle, but somehow we managed to get it)

mxc, thanks for the SSN part... I gaurantee you, my wife will not remember her SSN there :) I will make sure I have her SSN card with me.

But I am not sure if she will go with me next week because her FP2 is scheduled in December last week. It really sucks as I was hoping to get both FP2 together. I do not want to mess up in some way by doing her FP2 with mine (next week) when it's scheduled for next month. Or shall I take the chance/risk and try to have her FP2 with me? Dilemma dilemma dilemma!
 
My wife and I got our FP's done a full three weeks ahead of schedule. Well I don't know if that was a risk or not. Frankly I was not worried. The worst that could happen is, the officer that works on my application gets mad and puts me back to where I am scheduled to be or shoves the application under a pile. But I was willing to take the chance and believe that there is still some sense of responsiblity out there and nothing of that sort would happen. I have heard that people have done their FP early and have gotten approvals too. So, I just took a chance and definitely for a valid reason - my EAD and AP are up for approval and I need to apply atleast by mid Dec, so I was hoping if I can get my FP done atleast that gives me a theoritical chance of approval and may be I could save some money. But going by what's happened (actually nothing has changed) in the 10 days since I did my FP, I don't think I will see approval. Anyway, I did what I did and hope it will all end well.
