Sample for affidavit of bith certificate for parents

Hi everyone,

I have filed GC for my parents in sep 2004. Can someone send me the sample used for affidavit for birth cert for parents. My grandparents have expired and hence they cannot give the affidavit. Who can give the affidavit if the parents are not there.

I would appreciate if someone could provide me the sample and info on who could sign that.

I have rea somewhere that relatives older than your parents can give affidavite but please make sure with someone else too.
 
this is the one I used

for my parents, it was notarizaed and signed by my dad's uncle & aunt, I don't know if is ok because I just send it, as a response for RFE, anyway here it is:

I, John Doe, am familiar with the birth of Jim Smith. Jim Smith was born in Washington, DC, USA, on September 12, 1970. His mother’s name is Martha Smith. His father’s name is William Smith. I have known Jim Smith since birth. I know him to be the person specified in his passport. Passport copy attached.


I, John Doe, certify under penalty of perjury under the laws of the United States that:

(d) I know the contents of this affidavit of proof of birth signed by me;
(e) All the statements in this affidavit are true and correct;
(f) I make this affidavit stated above, freely, and without any mental reservation;

Anyone else has any other format?

Thanks for the format. But it looks like the format was done by some one born in US for a person in India.

I do not have any uncle/aunt in US. So, if my uncle/aunt who live in India sign this document?

murali is ok for your relatives to be foreigners but they have to undestand what they are signing and do it in fron of a notary, in other words, traslate the document to their native language fil it with they names and dates and send them over there, they take it to the notary and send it back and have it traslate it
 
Worng Name in Birth Certificate

Hi,Guys,

My sister got her US passport few days a go. Now she wants to file for my parents. Now she found that the birth certificate she subitted for her green card and citizenship has spelling error for my dad's first name and the last of mom's is different. How she can correct those names.

Please share your experience or thoghts.
