Salary and AC21

My LC job designation is Software Engineer and I offered a very attractive position what I was dreaming from years ..........but the designation J2EE/Enterprise Architect also significant jump in salary as well around 25K.
But my job responsibilities is about the same.

Can I go and file for AC21?

If so how can I file I mean which form I need to download?

in the same boat

i too got similar offer but did not file AC21 and also my question is
my labor got filed in Sep,1999 does that mean that do i need to work
for companies existing since then or can i join any recent (2002 year)dot com companies too. I mean, do i need to show proof to INS the ability to pay by the employer since 1999. sometime back my lawyer mentioned that but recently he said it is probably okay. and talked to a lawyer from murthy's firm
who said he never heard of such thing i can join any company as long as they can pay me but pinging for your opinion too
